Better health is the goal. Not everyone can be perfectly healthy. But, even with a chronic condition, you can reach your own best health. It’s not just about managing your condition, it’s about helping you live your life in the best way possible.
The Healthy Living Informed Care Management program is a condition management program provided by ActiveHealth Management, a wholly owned subsidiary of Aetna. The program is designed to help you better manage your health and take active steps to work with your doctor(s) to help improve your care. Your health information is kept confidential and not shared with your employer. Informed Care Management does not take the place of your doctor. It is, however, a great resource that will help you and your doctor make informed decisions about your health.
Here’s how it works:
- You are
identified as eligible for participation through:
- Referral by your physician
- Self-referral by calling 1-800-967-4490
- Medical and pharmacy claims data
- Your Health Assessment score
- You receive program information in the mail. You’ll receive a reply card that you can return to indicate that you’d like a nurse care manager to call you right away. You may also decline participation in the program.
- After two mailings,
a nurse care manager will call you to
see if he or she can help you manage your health issues better.
Your nurse care manager can:
- Review your health information and help you better understand your condition(s).
- Prepare a personalized care plan just for you.
- Suggest questions to ask your doctor(s).
- Talk with you about warning signs and symptoms related to your condition(s).
- Work with you to identify ways to stay healthy and set health goals.
- Ask you about any over-the-counter medications you are taking or allergies you may have.
- Discuss lifestyle factors affecting your condition(s).
- Send you helpful education materials about your condition(s).
After the call, your nurse care manager sends a follow-up letter that reminds you of points you discussed and steps you decided to take between doctor visits. You can learn how to manage your condition and what you can do to help protect yourself from future complications.
