Personal Health Record

At last – one place to find your medical history! No more digging through the filing cabinet looking for your children’s health records; no more searching through shoe boxes before you visit your doctor. Your comprehensive medical information can be right at your fingertips.

The Personal Health Record (PHR) is a single, secure online tool that automatically displays your medical information based on claims data and applicable Health Assessment responses. It also makes it easy for you to enter additional health information not captured by either, such as family health history and over-the-counter medications.

The PHR makes it easy to look up important health information by organizing the information and making it available with only a few clicks. You must enter a secure User Name and Password to access your PHR, so you control who sees it. You and each family member who participates in an eligible medical plan will have his/her own separate, secure record, and the plan subscriber will be able to access the PHR of each covered dependent under age 18.


You will be guided through a registration process to create a User Name and Password that will give you access to Simple Steps To A Healthier Life and the Personal Health Record. Be sure to enter your full, official name, the same as it appears in your insurance company files. (Aetna members can also access Aetna Navigator here. If you already use Navigator, you can use your same User Name and Password. You do not need to create new ones for Simple Steps and PHR.)
