The following examples illustrate how the Healthy Living programs might work for individuals with different health conditions.
Joe is a 56-year-old plant manager with diabetes and is overweight. He should test his blood sugar daily, have regular retinal eye / foot exams and reduce his weight. Unfortunately, Joe is taking none of these actions, even though they could slow the development of complications associated with diabetes. A nurse from the Healthy Living program contacts Joe to see if he is interested in developing a plan to help improve his health. After Joe consents, the nurse sets up an appointment convenient for Joe where they review his health history, current treatment and lifestyle. The nurse makes recommendations to help Joe reduce his health risks. She provides diabetes literature and an action plan for Joe to follow. She follows up with Joe to see how he is progressing and tailors the action plan to fit his needs.
Patricia is a 37-year-old accounting manager who smokes and leads a sedentary lifestyle. She completes the Health Assessment and is identified as having high blood pressure. This, combined with the smoking and sedentary lifestyle, puts Patricia at risk for heart disease. Patricia is contacted by a health coach from the Healthy Living program, who advises her of this, and suggests ways that Patricia can reduce her health risks.
Mary is a 52-year-old marketing manager who exercises, eats right and generally considers herself healthy. She is interested in obtaining healthy meal ideas for her family as well as an exercise program that will fit her busy lifestyle. The Healthy Living program provides exercise plans, as well as nutritional information (e.g., recipes, meal plans, etc.).
A 39-year-old accountant with high cholesterol, Jim takes Lipitor to lower his cholesterol. While completing his Health Assessment, it is discovered that he drinks grapefruit juice every morning. A nurse from the Healthy Living program contacts Jim, advises that grapefruit juice can reduce Lipitor’s effectiveness in reducing cholesterol levels and asks that he discuss this with his physician.
Susie is the 5-year-old daughter of Bob, an engineer. She was taken to the emergency room because she suffered an asthma attack and was admitted to the hospital overnight. A report was generated from the health management system indicating her parents should be contacted to discuss Susie’s treatment and enrollment in the condition management program. A nurse from the Healthy Living program contacted Susie's mother, who agreed to enroll with nurse education. The nurse obtained a list of Susie's medications and discussed with her mother things such as asthmatic triggers, allergy testing, flu and pneumonia vaccines and developing an asthma action plan. She encouraged her to discuss these things with Susie's doctor. The next follow up appointment with the nurse was set up and Susie's mother was mailed educational material on asthma.
