Access Instructions

• At the Aetna registration screen, enter your Aetna Navigator user name and password. If you are a new user, click Go under First-Time User, and you will be guided through a registration process.

• If you are a spouse, you must register as a dependent by selecting the dependent option indicated below.

• Under the Health Management section, select Assess Your Health with Simple Steps To A Healthier Life.

• In the My Health Assessment box, you may elect to either create a new Health Assessment (Health Risk Questionnaire or HRQ) or update an existing one.

• For updates, hit the Continue button at the bottom of page to save any changes and move forward. To qualify as an update, you must enter new information regarding your current health profile.

• You will receive a certification message upon completion of the HRQ. It will show the current date if you successfully completed or updated your HRQ. You may wish to print a copy for your records.
