Healthy Living Informed Care Management offers more than just condition management. The program uses advanced technology from ActiveHealth Management, called the CareEngine® System, that monitors data from your medical,
pharmacy and lab claims to help make sure you’re getting
the best and safest care.

Technology that promotes better care and patient safety
The CareEngine systems scan 100 percent of the population 100 percent of the time – comparing member health information with more than 1,000 evidence-based guidelines of care. It can identify potentially dangerous drug-drug interactions, drug-disease interactions and the need for preventive screenings or other care.

Outreach that helps improve your care and safety
If the CareEngine notices a situation that might harm you or that a more effective treatment may be available, the system issues a Care Consideration. Care Considerations notify your doctor to consider:

No one doctor can be expected to know a patient’s information from across so many spectrums of care. Your physician will benefit from improved information about each patient, and members and doctors can share that information to make the best decisions concerning their health care.
