Do you ever feel like you’re on a roller coaster? You start with the best intentions, planning to finally lose those extra pounds or quit smoking for good. But, after having a couple of days or even weeks of success, you find yourself slowly falling back to your old habits.

Your solution is here. Welcome to the Healthy Lifestyle Coaching program for weight management, tobacco cessation and healthy living. These confidential programs can help you make healthy changes that last.

This program replaces the Wellness Counseling program. Just like Wellness Counseling, Healthy Lifestyle Coaching provides one-on-one support for helping you meet your healthy living goals, such as eating better, getting in shape and reducing stress, however, the program offers extra help for weight management and tobacco cessation, as shown in the chart below. Give it a try. What have you got to lose…other than some bad habits!

Get personal support that meets your needs.
When you start one of the programs, you’re matched with a personal health coach who is trained to understand your needs. You’ll talk with your coach privately, over the phone, when it’s convenient for you. Together, you can:

Work one-on-one with your coach.
Your coaching sessions will vary depending on what category you fall into. Here’s how it works:

If any of the following describes you…

Then you get…

  • Your health assessment score is 50 or higher.
  • You use tobacco.
  • Your body mass index is 25 or higher.
  • You are referred from another Healthy Living program, such as Condition Management.
  • Kickoff session: One session for up to 30 minutes with your primary health coach to outline the program and get to know each other.
  • General assessment session: One session for up to 30 minutes with your primary health coach to complete a general assessment and review medical history and any medications.
  • Targeted session: Based on your general assessment session, you may then have one or more of the following sessions:
    • Motivational coaching: Weekly 30-minute coaching sessions for up to 30 minutes each for up to one year with your primary health coach.
    • Weight Management: Motivational coaching sessions as described above PLUS 8 sessions for up to 30 minutes each with a team of registered dietitians, nurses and certified personal trainers. You are eligible for one series per year.
    • Tobacco Cessation: Motivational coaching sessions as described above PLUS 1 pre-quit session for up to 50 minutes per quarter.
  • Your health assessment score is less than 50.
  • Kickoff session: same as above
  • General assessment session: same as above
  • Motivational coaching sessions: Up to 2 additional coaching sessions with your primary health coach.

Note: If you participate in the Tobacco Cessation Program, your coinsurance will be lower
for certain smoking cessation drugs.
